 Basketball Cheerleading Tryouts. 

 

FJH WINTER/BBALL CHEERLEADING TRYOUT TIMES – Tuesday, October 27

7TH Grade Group 1 RED and ALL WEEK students (who attend school on Tryout Day) can stay after school in the cafeteria until their 3:30 time slot, all others should go home as normal and come back at your designated time

(assignments by last name, you will try out in groups of 2 or 3)

7TH

8TH

Group 1 A-E:  3:30-3:45

Group 2 F-L:  3:45-4:00

Group 1 A-B:  4:30-4:45

Group 2 C-H:  4:45-5

Group 3 M-S:  4:00-4:15

Group 4 T-Z:  4:15-4:30

Group 3 I-Q:  5-5:15

Group 4 R—Z:  5:15-5:30

Reminders:

  • All paperwork, instructions, and videos of tryout material are on the
    Cheerleading web page
  • Know your material!  You won’t know until that day who you are trying out beside...if they don’t know their material it can really throw you off…make sure you can’t be thrown!
  • Follow up with your recommendations BEFORE tryouts…missing recs can keep you from making a squad
  • You must have a physical on file and your application submitted to try out (if you are unsure – ASK!  E-mail Coach Forman at hforman@hse.k12.in.us
  • Dress according to the tryout criteria
  • Enter the Main Office Door #1 and wait in the hallway between the LGI and the Cafeteria (maintain social distance), arrive at least 5 minutes before your tryout time
  • Rides should wait if possible, you will be done in 20 minutes or less!
  • You will be given an opportunity to warm up tumbling

Squads will be posted on the FJH Cheerleading web page, by number (assigned at tryout), by 6p

 


Medical Forms Needed for Participation in Athletics

1. IHSAA Sports Physical - hard copy on file with Athletic Director - physicals on file from the 2019-2020 school year WILL BE VALID for 20-21. 

2. Rank One Online Signature Forms - submitted electronically - must be completed each year.

3. Health History & Consent Form - NEW for 20-21 - submitted electronically - Required for ALL athletes. This is new this year from the IHSAA and can be done electronically online with the link above. If an athlete answers YES to any of the seven (7) questions on the Health History Questionnaire, they must get a new 2020-21 IHSAA Athletic Physical and turn it in to the athletic office prior to returning to play.

4. ECA Consent & Release Form - NEW for athletes pariticpating in any fall sport this year. -submitted electronically.

     b. Athletes & parents must both sign the bottom of the last page

     c. The form is done electronically (link above)

***For the 2020-2021 school year only: Valid physicals from the 2019-2020 school year WILL be valid for 2020-2021. If you did not have a physical on file for the 2019-2020 school year, you WILL need to turn in a physical dated on or after April 1st, 2020 in order to participate during the 2020-2021 school year.The most updated IHSAA form will be the only form accepted. The form can be found on our athletics webpage. Please check each sport’s website for important dates so your child does not miss their opportunity, as many of our fall sports begin prior to the first day of school.***

Medical Forms Needed for Participation in Athletics

IMPORTANT INFO REGARDING IHSAA SPORTS PHYSICALS - PLEASE READ ALL!!!

IHSAA Physical: Print off and complete the IHSAA Physical Form.  Any 7th, or 8th grade student who wants to participate in any school sport during the 2017-2018 school year MUST have an IHSAA sports physical on file with the Athletic Director before participating. Physicals may be turned into the athletics office or to the coach on the first day of tryouts. The physical must be dated on or after April 1st, 2017. The most updated IHSAA form will be the only form accepted. 
 

  • The form must be signed by a physician (MD or DO),Nurse Practitioner, or Physician's assistant on page 2 of the form.

  • The physician's license # must be included on page 2. 

  • NO stamped signatures will be accepted.

  • Student-athlete & parent must sign bottom of page 1 and page 4. 
     

  • DO NOT EXCLUDE ANY PAGES OF THE PHYSICAL!

Ticket / Admission Information

Admission prices for the following sports: Football, Volleyball, Boys/Girls Basketball, Wrestling and Track. These prices are good for home athletic events at FJH but not for tournaments.
