Medical Forms Needed for Participation in Athletics

1. IHSAA Sports Physical - hard copy on file with Athletic Director - physical exams must be completed in the late spring or summer in order to be eligible for the following academic year.

2. Rank One Online Signature & Consent to Treat Forms - submitted electronically - must be completed each year. PLEASE WAIT UNTIL AFTER JUNE 6th TO COMPLETE THESE!

Any 6th, 7th, or 8th grade student who wants to participate in a school sport during the 2022-2023 school year MUST have an IHSAA sports physical on file with the Athletic Director & Rank One online forms submitted before participating. This includes any practices, camps, open gyms, or conditioning that may take place over the summer.

*The form must be signed by a physician (MD or DO), Nurse Practitioner, or Physician Assistant on page 3. 
*The physician's license # must be included on page 3. 
*NO stamped signatures will be accepted.
*Student-athlete & parent must sign bottom of page 2 and page 5. 

*DO NOT EXCLUDE ANY PAGES OF THE PHYSICAL!

 Club and intramural sports programs do not require a physical. 

Ticket Information

Admission prices for the following sports: Football, Volleyball, Boys/Girls Basketball, Wrestling and Track. These prices are good for home athletic events at FJH, but not for tournaments.

All-Sport Pass (single) -- $50.00
All-Sport Pass (family) -- $100.00
